Zepbound Cost Without Insurance: What You Pay in 2026 by Strength and Refill Timing

Your Zepbound cost without insurance depends on two things: the strength on your prescription and how quickly you refill it. Buying directly from Eli Lilly through LillyDirect, a 28-day supply costs $299, $399 or $449 in October 2026, and up to $699 if a refill of the higher strengths comes late.

This guide puts every published self-pay price in one place, shows what a year adds up to with our own worked examples, and lists the mistakes that make people pay more than they need to. Every price links to Lilly’s own page and carries the date we checked it. Our How We Verify page explains how we check each figure.

Zepbound cost without insurance at a glance

The table shows the Zepbound cost without insurance at each strength, using Lilly’s self-pay prices for a 1-month supply, which Lilly defines as 28 days: 4 single-dose vials or 1 KwikPen. The vial and the KwikPen cost the same at each strength.

Where the $299 to $449 prices come from

The lowest Zepbound cost without insurance comes from Lilly’s own self-pay channel. Your prescriber sends the prescription to LillyDirect Pharmacy, and one of the dispensing pharmacies Lilly works with contacts you to confirm the order and take payment. Lilly names four of them: Amazon Pharmacy, Fuze Health, Gifthealth and Walmart Pharmacy.

Lilly calls the discount on the higher strengths the Zepbound Self Pay Journey Program. According to Lilly’s terms, new and existing patients receive the $449 price automatically on the first purchase of a 7.5 mg, 10 mg, 12.5 mg or 15 mg supply. You do not need a coupon or a code.

Three details change the final amount you pay:

    • Supplies are extra. Vials need a new needle and syringe for each injection, and the KwikPen needs a new pen needle. Lilly sells them at checkout, but they are not included in the prices above.
    • Taxes and fees may apply. Lilly’s terms say so next to each regular price.
    • Delivery is free. Lilly offers free home delivery or free pickup at a Walmart Pharmacy.

The list price that Lilly charges wholesalers is a different number. Lilly publishes it on its Zepbound pricing page and notes that what patients pay at a pharmacy can differ from it. The self-pay prices in this guide are the ones Lilly sells to cash-paying patients.

The 45-day refill rule, in dollars

The lowest Zepbound cost without insurance on the higher strengths, $449, has one condition. Lilly’s terms say you must complete each refill purchase within 45 days of the date your previous supply was delivered or received. If you buy later than that, the regular price applies to that fill.

A 28-day supply leaves 17 days of margin before the 45-day window closes. That margin is useful if a delivery is delayed, but a missed reminder or a pause in treatment can push a refill past it.

StrengthOn timeLateExtra cost of one late refill
    7.5 mg$449$499$50
    10 mg$449$699$250
    12.5 mg$449$699$250
    15 mg$449$699$250
Our calculation from Lilly’s published prices, checked Oct 18, 2026. The 2.5 mg and 5 mg prices do not depend on refill timing.

    Worked example: one late refill

Say a 10 mg supply arrives on March 1. To keep the $449 price, the next purchase has to be completed by April 15, which is 45 days later. If it is completed on April 20 instead, that fill costs $699, or $250 more. The fill after that is back at $449 as long as it is completed within 45 days of the April delivery. One late refill is the quickest way for your Zepbound cost without insurance to jump in a single month.

Yearly Zepbound cost without insurance: worked examples

To estimate your yearly Zepbound cost without insurance, count fills, not months. Because Lilly prices Zepbound by the 28-day supply, a full year of refills is 13 fills, not 12. That detail adds one extra month to every annual estimate, and it is easy to miss when you multiply a monthly price by 12.

Yearly Zepbound cost without insurance for 13 fills, from $5,637 to $9,087 depending on refill timing
    ScenarioHow we calculated itYearly total
    A. Every fill at $449, all on time13 × $449$5,837
    B. First fill at $299, second at $399, then $449 on time$299 + $399 + 11 × $449$5,637
    C. Every fill at 10 mg or above, all late13 × $699$9,087
    D. Like A, but two fills late at 10 mg or above11 × $449 + 2 × $699$6,337
Yearly Zepbound cost without insurance, our calculation; prices checked Oct 18, 2026. Supplies, taxes and fees are not included. The strength in each scenario is an assumption for the arithmetic only: the strength you use is decided by your prescriber.

Scenario C shows why the refill rule matters most at the higher strengths: paying the regular price all year costs $3,250 more than scenario A. Scenario D shows that even two late refills add $500.

Spread over 364 days, scenario A puts the Zepbound cost without insurance at about $16.04 a day. That figure is useful when you compare it with other monthly costs in your budget, but it does not include supplies.

Vial or KwikPen: does the form change the price?

Not at LillyDirect: the form you choose does not change your Zepbound cost without insurance there. Since February 23, 2026, Lilly’s terms set the same self-pay price for the single-dose vial and the KwikPen at each strength. The differences are in where you can buy each form and what supplies you need.

If you fill a KwikPen prescription at a retail pharmacy instead of LillyDirect, Lilly offers a separate Zepbound KwikPen Self-Pay Savings Card with the same “pay as low as” prices. Its terms add three limits worth knowing:

    • The card can be used for up to 11 fills per calendar year, while a year of 28-day supplies has 13 fills.
    • The card expires and savings end on December 31, 2026.
    • If the pharmacy cannot confirm your 45-day refill at the counter, you may have to claim the difference afterwards. Lilly’s terms set a deadline of March 31, 2027 for those claims.

Lilly’s terms do not say what fills 12 and 13 cost at a retail pharmacy once the card limit is reached. If you plan to use a retail pharmacy all year, ask the pharmacy or Lilly before you reach that point.

Zepbound cost without insurance compared with other ways to pay

Lilly publishes four price routes for Zepbound. The table puts the Zepbound cost without insurance next to the other three, so you can see where you fit.

RouteWho it is forWhat you pay for 28 daysMain limits
LillyDirect self-pay (vial or KwikPen)Anyone with a valid prescription who pays cash$299 to $449 on time; up to $699 late45-day refill rule on 7.5 mg and above
KwikPen Self-Pay Savings Card at a retail pharmacyCash-paying patients with a KwikPen prescriptionAs low as $299 to $449Up to 11 fills a year; expires Dec 31, 2026
Savings card, commercial plan that does not cover Zepbound (single-dose pen)Privately insured, no Zepbound coverageAs low as $499Up to 13 fills a year; expires Dec 31, 2026
Savings card, commercial plan that covers Zepbound (single-dose pen)Privately insured with coverageAs low as $25 for a 1-, 2- or 3-month fillSavings up to $100 a month and $1,300 a year
Source: Lilly’s full terms and conditions, checked Oct 18, 2026. Government plans, including Medicare and Medicaid, are excluded from the savings cards.

Among these published routes, the lowest Zepbound cost without insurance is the LillyDirect self-pay price, matched by the KwikPen card at a retail pharmacy. The $25 price needs a commercial plan that covers Zepbound, and the savings behind it are capped, so the amount you pay can be higher once your plan’s share and the caps are counted.

If your insurance does not cover Zepbound

If you looked up the Zepbound cost without insurance because your plan refuses to pay for it, Lilly’s savings card terms describe two options for commercial (private) insurance that does not cover Zepbound:

    • Single-dose pen: pay as low as $499 for a 1-month fill, for up to 13 fills a year.
    • KwikPen: pay as low as $299, $399 or $449, the same tiers as self-pay, for up to 11 fills a year.

Both cards expire on December 31, 2026, according to Lilly’s terms. Over 13 fills, the single-dose pen at $499 comes to $6,487 a year, which is $650 more than scenario A above (our calculation).

Government coverage is a different case. Lilly’s savings cards exclude anyone enrolled in Medicare, Medicaid, TRICARE, VA or another government-funded program. For Medicare Part D, Lilly points eligible patients with a KwikPen prescription to its Medicare GLP-1 Bridge program, which Lilly lists at $50 a month and valid from July 2026 to December 2027. Eligibility rules and prior authorization apply, and pen needles are extra. We will cover Medicare in a separate guide.

5 costly mistakes that raise your Zepbound cost without insurance

Much of the gap between a low and a high Zepbound cost without insurance comes from habits rather than from the price list. These five come from Lilly’s published terms and our own calculations:

    1. Refilling after day 45. At 10 mg and above, each late fill costs $250 more. Put the delivery date in your calendar and set a reminder well before day 45.
    2. Budgeting for 12 months instead of 13 fills. A 28-day supply means 13 purchases a year. Scenario A is $5,837, not 12 × $449 = $5,388.
    3. Forgetting supplies. Needles, syringes or pen needles are bought separately. Add them to your monthly total before you compare options.
    4. Using a third-party “coupon” instead of Lilly’s own program. Lilly’s card terms say the savings cannot be combined with any other discount card or coupon. Start from Lilly’s official pages linked in this guide.
    5. Assuming today’s terms last forever. Lilly can change or end these programs at any time, and the current cards expire on December 31, 2026. Check the price again before each new year.

Paying with an HSA or FSA

If you plan to pay your Zepbound cost without insurance from a health savings account (HSA) or flexible spending account (FSA), start with Lilly’s own note. Its LillyDirect page marks the vial, the KwikPen and the single-dose pen as HSA/FSA eligible, with the note that eligibility may vary by plan. Check with your account provider before you pay. Lilly’s savings card terms also say you may not seek reimbursement from a health savings or flexible spending account for the amount the card saved you.

    Frequently asked questions

How much is Zepbound without insurance per month?

Through LillyDirect, the Zepbound cost without insurance for a 28-day supply is $299 at 2.5 mg, $399 at 5 mg and $449 at 7.5 mg to 15 mg when you refill within 45 days. Late refills of the higher strengths cost $499 or $699. Prices checked Oct 18, 2026.

What is the cheapest Zepbound cost without insurance?

Among Lilly’s published prices, it is $299 for a 28-day supply at 2.5 mg through LillyDirect. At 7.5 mg and above, the lowest Zepbound cost without insurance is $449, as long as each refill is completed within 45 days.

Is the Zepbound vial cheaper than the KwikPen?

No. Since February 23, 2026, Lilly charges the same self-pay price for both forms at each strength. The vial needs needles and syringes, and the KwikPen needs pen needles, so compare the cost of supplies too.

Do I need a coupon code for the self-pay price?

Not at LillyDirect. Lilly says savings are applied automatically there, and the $449 price on the higher strengths is applied on your first purchase. At a retail pharmacy, the KwikPen self-pay price needs Lilly’s activated savings card.

Can I use Lilly’s savings card with Medicare?

No. Lilly’s terms exclude people enrolled in Medicare, Medicare Part D, Medicare Advantage, Medicaid and other government programs. Lilly points eligible Medicare Part D patients to its separate Medicare GLP-1 Bridge program, listed at $50 a month for the KwikPen.

Will these prices change in 2027?

Your Zepbound cost without insurance may change. Lilly’s current savings cards expire on December 31, 2026, and Lilly says it can change its programs at any time. We review every price on the 10th of each month and log changes, so check the date next to each figure.
